Let’s Get You Enrolled
Once your student has been selected through our lottery, the next step is completing enrollment. Please gather the documents below before submitting your registration. If you have questions or need help locating anything on this list, reach out to us and we will help you figure it out.
Required Enrollment Documents
- Official document showing proof of birth date (birth certificate or certified birth record)
- Current proof of residency (mortgage or lease and a utility bill showing the name of the parent or guardian and current address)
- Notarized and complete Affidavit of Residency, if applicable
- Valid Georgia Certificate of Immunization (GA Form 3231)
- Evidence of Vision, Hearing, Dental, and Nutrition Screening (GA Form 3300)
- Valid government-issued photo ID of the adult registering the child (current driver’s license, military ID, or passport)
- Child’s Social Security card or number (or you may sign a waiver)
- Most recent report card or official school transcript
- Most recent discipline records from previous school
- Withdrawal documents from previous school
- Tapestry supplemental documents (provided at your enrollment appointment)

Certificate of School Enrollment
Need proof that your student is enrolled at Tapestry? We can provide a Certificate of School Enrollment. Please allow 2 business days for processing. Certificates are valid for 15 to 30 days from the date of issuance.
To request a certificate, email with the following:
- Student’s full name and date of birth
- Home address
